Retatrutide is a single-peptide agonist at three receptors: GIP, GLP-1 and glucagon. Adding glucagon-receptor activity to the incretin mechanism is hypothesised to increase energy expenditure alongside reduced intake.
In a 48-week phase 2 trial (n = 338), retatrutide 12 mg produced a mean weight change of −24.2% versus −2.1% with placebo, with a dose-dependent response across the 1–12 mg range 1.
